Long Term Conditions

Long-term conditions are chronic diseases or conditions for which there is currently no cure, and which are managed with medications and other treatments.

These conditions can generally last for a year or longer and may impact upon a person’s everyday life.

Examples include:

• Asthma
• Chronic Obstructive Pulmonary Disease (COPD)
• Chronic kidney disease
• Coronary heart disease
• Diabetes
• Epilepsy
• Heart failure
• Hypertension
• Multiple Sclerosis
• Parkinson’s disease
• Stroke

What should I do to manage my condition?

Everyone’s experience of living with a long-term condition is different and you may require regular reviews with one of our team members to help manage your condition.

Lung Cancer Screening Service

“If you’re aged 55 to 74 and have ever smoked, you may soon receive a letter from InHealth inviting you to take part in the Lung Cancer Screening Programme.

We want to reassure you that this invitation is genuine. InHealth is working alongside Orchard Surgery and the wider NHS to offer free lung health checks and encourage eligible patients to take part.

These checks help detect early signs of lung cancer, when treatment is most effective. Attending your appointment could make a real difference to your health.
