You can also come into the surgery during the opening hours to complete our registration forms. You can download and complete the NHS GP patient registration form.

Please return your completed registration forms to reception or email or post to us. If you don’t have an access to emails or a printer and would like us to post a registration form to you please give us a call on 01256 479 747.

We aim to process your registration within 48 working hours.

Safe Surgeries Initiative

We are a Safe Surgery – everyone in our community is welcome at our practice.

Everyone living in England has the right to free care from a GP and we work hard to make sure no one in our community is excluded from healthcare. This means that:

  • When you register with our GP Practice, we might ask for ID or proof of address.
  • However, if you don’t have any and you live in our practice area, you can still register with us.
  • Your nationality or immigration status does not affect your right to register with us.
  • We won’t ask you for immigration documents when you register at our practice.
  • If you are worried about giving us your address, please let us know. Your information is safe with us.

Thinking of registering with Park Surgery but live outside our catchment area?

Practices can choose whether to register patients who live outside their practice boundary. If we do accept an out-of-area registration, it is without home visits and some urgent in-person care.

Before you start, please read the important points below so you can decide if this is right for you.

Important things to consider Depending on where you live, you may experience:

  • Difficulties arranging or coordinating referrals to specialist/consultant services in your local area.
  • Delays where blood tests and other investigations done locally do not flow automatically to our system.
  • Park Surgery clinicians (GPs, nurses, paramedics, physician associates) and district nurses will not be able to provide home visits to your address.
  • Community services: We will discuss options and make referrals, but local community services may have access barriers for patients registered outside the area.

What this means for your care

  • We will provide core GP services, but the limitations above may affect continuity and timeliness of your care.
  • If your health needs change (for example, increasing need for home or community support), it may no longer be clinically appropriate or practical for you to remain registered out-of-area, and we may ask you to register with a local practice, in line with NHS contract rules.
